Diabetes Group Visits: Moving beyond the one-on-one office visit


Group visits have been implemented in a variety of clinical settings, including fee-for-service practices, managed care clinics, and even academic settings. This resource is a suggested 6-month curriculum for diabetes group visits. It includes the potential structure, format, and information to provide during group visits. It also gives useful information on how much staffing may be required, how to bill, and how to incorporate medical students and residents. Useful handouts to track patient outcomes, as well as invitation letters and consent forms, are also included. Suggested surveys are also included to measure the success of the program.
